Photo Release
Greater Cleveland Junior Golf Scholarship Fund, Inc. — Junior Golf Summer Program (optional)
This release is optional. You can register your child without granting it; the program experience is identical either way. Leaving it unchecked means GCJGSF won't include photos of your child(ren) in promotional materials.
Permission Granted
By checking the photo release checkbox on the registration form, I grant Greater Cleveland Junior Golf Scholarship Fund, Inc. and its authorized representatives permission to photograph, videotape, and audio-record my child(ren) during program activities. I further grant GCJGSF permission to use those photos, videos, and recordings for:
- Marketing and promotional materials (website, social media, brochures)
- The annual GCJGSF Impact Report
- Donor newsletters and fundraising campaigns
- Press releases and community-newspaper outreach
- Internal organizational records and program documentation
Limits
GCJGSF will never (a) sell photos of program participants to a third party, (b) identify minors by full name in any public material without separate written consent, or (c) use photos in contexts that conflict with the program's mission.
Withdrawing Consent
You may withdraw this consent at any time by emailing info@gcjgsf.org. After withdrawal, GCJGSF will remove the participant's images from future marketing materials. Previously-distributed materials (printed brochures, archived website pages, third-party news outlets) cannot be retroactively edited.
