Liability Waiver

Assumption of Risk and Release of Liability

In consideration of the opportunity to participate in GCJGSF's junior golf program, I, on behalf of myself, my child(ren), and our heirs, executors, administrators, and assigns, hereby acknowledge and agree to the following:

Assumption of Risk

I understand that participation in golf instruction, play, and related activities involves inherent risks, including but not limited to: physical injury from being struck by golf balls, golf clubs, or other equipment; falls or accidents on the course; weather-related hazards (sun, heat, lightning); transportation between facilities; and other risks both foreseen and unforeseen. I voluntarily assume all such risks on behalf of my child(ren).

Release

I hereby release, waive, and forever discharge Greater Cleveland Junior Golf Scholarship Fund, Inc., its officers, directors, employees, volunteers, instructors, partner golf courses (including Cleveland Metroparks and any other facilities used during the program), and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to my child(ren)'s participation in the program.

Medical Authorization

In the event of injury or illness, I authorize GCJGSF staff to seek and consent to emergency medical treatment for my child(ren) when I cannot be reached. I am responsible for any associated medical costs.

Code of Conduct

My child(ren) will follow program rules, instructors' direction, and standard golf etiquette. I understand the program may dismiss a participant for disciplinary reasons, and that no refund will be issued in that case (per the refund policy).

Acknowledgment

By checking the "I agree to the liability waiver" box on the registration form, I confirm that I have read this waiver in its entirety, understand its terms, and am signing it voluntarily on behalf of myself and my child(ren).
